Flock Health Attestation

Some poultry movements within, out of or into a Primary Control Zone (PCZ) require you to complete a Flock Health Attestation (FHA).

When you are completing an FHA, please ensure you fill in all fields. This includes indicating when a barn is empty, which you can do by writing the barn number and then “0” for bird population.

Reporting period should always be the previous 7 days, including the day-of information.
